The list of health care providers participating in
the Flora Health Network is subject to change
Flora Health Network is one of Ohio's
largest independent Preferred Provider Organizations (PPO).
Flora's sole purpose is to provide the uncompromising best in
reliability, quality and service. To serve this purpose, Flora has
dedicated itself to building one of the most comprehensive networks of
hospitals, physicians and other health care providers. We are
available to assist you in selecting a physician or hospital; however,
the final responsibility and freedom of choice rests solely with you.
For information regarding your coverage,
benefits or eligibility, please check with your employer or your
insurance company. Flora Health Network is not an insurance
company or third party administrator and does not process claims or make
payments. Please forward all claims to the address listed on your
medical ID card to ensure timely payments of your medical claims.

How to use your
PPO Network
|
To be certain you receive your full
benefits and that Flora's provider discounts are applied, be sure to:

Select a provider from the directory
or call Customer Service for assistance in locating a provider.

Confirm with your provider that they
are currently part of the Flora Health Network.

Remind the provider (physician) your
health plan uses Flora Health Network (PPO) and show your current
medical ID card.

If your provider needs to refer you
to a facility or another health care provider, remind the provider
that you are part of Flora Health Network and be sure the provider
refers you to a participating provider when possible. If you
use a non-participating provider, your benefits may be reduced.

When your medical plan is part of
Flora Health Network, you should have a Flora sticker on your
medical card. If there is not a Flora sticker on your card,
please contact your employer.

regions not included in your directory, or questions on membership,
please call Flora directly.
